Default SSDI CDR and received Functional Form

Hi all,

I have been on SSDI for 6 years. I am now in my first review. I filled out short form then a month later got a long form and just yesterday got a functional form to fill out. It is my understanding that not everyone gets this during the review. I am on SSDI for bipolar 1 mixed and OCD. Does anyone know if this is standard for a first review or should I be worried sick like I am now???

You need to treat the long form SERIOUSLY. It is equivalent to your initial application.

This link provides a good explanation of the CDR process.

You maybe got the long form because of how you filled out the form, or because you've gone back to school or started working part time, etc. It is important to show you regularly see your doc and take your meds as directed, and that your condition continues to be severely disabling.

wheeler,usually it's either the short form or the long form that you recieve and not both..the short form is sent to people who's disabling condition isn't likely to improve and the long form is for people who's conditions are expected to improve.have no idea why they sent you both.
