Question I have 1st appt w Neurologist Mon.Questions

I'm still working on my SSD application and will tell my story later when I feel better.

My insurance ends this month so I'm seeing him Monday and he's doing EMG Wed. My Dr thinks I have PN and so do I. Since my ins is ending I wont be able to see him again,unless it's before the end of this month. So on this first visit,should I go ahead and tell him I'm filing for SSD and ask him if he will fill out the form I need,when the time comes,if he finds something,or should I wait and see what he says about the EMG and what his DX is and just send him the forms to fill out.

I feel like I should tell him a little of my history,etc,and why I'm filing so he will know. What should I do?
