Default What's the name of this form?

Hi. Every year my husband and I get this statement from the Social
Security Department which states how much we got for the whole year. We always bring this to the tax guy (even though we know we don't have to file) but in order to get an NYC Earned Income teachers rebate (or something to that effect, it's about $65 each), we file at the tax guy.

We can't find these statements. Alan put them someplace and I can't find them. I want to get another two statements but I can't reach the Social Security department on the phone and I don't know the exact form number so I can do this online?

Can someone please give me the form number so I can apply online so they can remail me our statements?

1099 is what you want. Not sure you can get it online but the folks at the 800# can print and mail you a replacement - won't look as nice but it will have all the data.
