Default just recieved my short form disibility report

hi folks,i just got in the mail a few days ago my disability update report to fill out.last one I filled out was about 3 years ago.this is the 6 question report.some examples of the questions that they ask are as follows....are you working?...how do you feel? better,same,worse..have you seen your doctor recently?any surgeries since the last update???..just to name a few....the short form disability report are sent to those people who's disabling conditions's are not likely to improve where as the long form which is several pages in lenghth is sent to those who's disabling conditions are likely to improve.

I think i screwed mine up i had help and we changed my phone number on front page, now it will get "kicked" out and be given to a human and then i will probably get long form idn what to do, nothing to do i guess
