Default need help doc white washed me...what to do?

I am a 57 y/o pharmacist with 2 concussions and have not worked since 12/12. I am being treated at Standford for post concussion syndrome. early aug I went to my reg doc to get cholesterol checked. I fired my first pcd because he totally missed my 1st concussion after my car accident and 10 days later I walked into a poll. anyway I went into a total profanity rant about him (personality change, unable to maintain correct social skill ect) she was asking so many questions I forgot why I was there ( confusion). she new me 20 min and wanted to change my neurologist and psychologist meds. I was overwhelmed.
next day I was totally depressed and stayed in bed. I typed a full page that answered her questions. my 7 types of headaches. why I wanted to stick with the specialist, ect. made an apt for the next day.
following day. apologized. offered to leave her practice. gave her my letter that was scanned into the record. then came the lecture, since I wasn't working I should got to the beach and stand on one leg and move my arms slowly to the sun. I tried. I really did. just kept telling myself let her talk. then said im not really interested. she just does not under stand post concussion syndrome. I am not able to get up every day let alone stand on one foot. I left with my cholesterol test and a referral for a colonoscopy. lol. fine what ever.
however here is the rub.
when I got my records all it says is I came in. we talked. blah blah blah. my scan does not show, my second appointment is listed as a follow up.
ssa will be getting two totally different stories. one from me and one from the doc. I am thinking of going to talk to the office manager. what should I do? this is really going to hurt my case.

ps. I had a ssa psyc exam a few days ago. I was looney toons. my friend got too go into the exam with me, I asked the person that took me if I was in my car or her car after about 3 minutes in the car. I answered I drove to the exam when she drove. I randomly shaved my head the night before the exam. my friend told all this to the examiner. the look on the examiners face was priceless. at the end I asked if she wanted me filling her prescriptions. she said NO!

could really use some help with getting my doctors record straight. just want the truth.

You need to go back to the doctor and point out the errors in the report. Either the doctor will do a supplemental report or write up another. Either way, get copies. While you're at it, also ask for copies of the doctor notes.

Bring a friend (witness) in case you cannot make yourself understood.

Your 'loony toons' demeanor at the psych CE; was that an act or the real you? You don't want to lose credibility in the eyes of SSA.

sadly it was real. my family is not happy especially my adult sons. I do not handle stress well these days. personality change, hostility are classic for post concussion syndrome.

Do you also have medical documentation from your doc/docs at Stanford that backs up what you wrote on your application ?

ya.. totally functional award winning pharmacist till two concussions in December. everything is documented. now basically wacked. full blown pcs. just check all the symptoms. most severe. noise sensitivity, anger, rage. memory loss. shaved my head... look like "breaking bad" walking around with gun ear muffs on. saying nasty word you.... all the time

Good. Well, you know, NOT good that you've suffered such losses, but good that some of your medical providers support your claim and have documented everything well..
