Default Error in Filling Out CDR

In filling out my most recent scannable short form CDR I failed to list the doctors' visits starting with the most recent one. I listed three different visits starting with the one the longest ago. Just failed to read the instructions correctly. Is this anything I should worry about? Should I request a new form or will this cause additional problems? Other advice?
