I am in the initial application process for my SSD.
My previous employer called me saying that he has received another form from the SSA requesting more information. He already completed the SSA-3033 form which has to due with subsidy. I'm trying to figure out what this other form is for and why they sent it to him. I wasn't able to ask him what form number it was before our conversation was cut short. Can anyone give me an idea what form this might be? Thanx, Rae PS: This would have nothing to do with employer/company disability insurance. This company did not carry disability insurance. |
