I received a CDR-455 short form. I recently had surgery to remove a cancerous spot on my ear so I listed that under question number 5 which asks "Have you been hospitalized or had surgery..". I probably should not have listed it because it has nothing to do with my disability in any way shape or form, as I am on disability for depression/mental health issues. Will this automatically trigger a review for me?

This is an old thread and the OP was answered in other threads, but in case someone else reads this, it's important that they fill their CDR out properly to avoid the dreaded long form CDR--which can be similar to the initial application process. Also, factors such as the probability for improvement and the beneficiary's age determine how often CDR's are sent. It can be as often as once a year, or every 3-4 years, etc. |
