Default Were my CDR Short Form answers wrong?

Hello everyone,

I received the CDR short form recently and answered the questions honestly and returned the form the next day.

My fears are that I may have messed up the answers I put on the form and it may get "kicked out" and trigger a full blown 10 page CDR. I read all the advice on this forum (and others) and I didn't add any new information to the form but my concern is question #5 for recent Dr. visits (I go regularly once every month or two months to my psychiatrist). Instead of putting the original reasons for my disability for the last three visits I put "Prescriptions" "Counseling" and "Prescriptions." These were the suggested answers on the form that they had listed ("Have you gone to a Dr or clinic for evaluations, checkups, counseling, prescriptions or medicine?"). And those were the reasons I went to my psychiatrist for my last three appts. I went to get prescriptions filled and also for counseling. Should I have put in my original diagnoses like "Bipolar I" or something like that? Did I mess this up?

Also I didn't look up my codes at the top of the form or keep a copy so I don't even know what my original codes were, I just have a really long psychiatric history of hospitalizations for bi polar, depression and suicide. Do you think my form will get kicked out for review? Everything else I answered was normal: no work, no training, Dr. says I can't work, condition is same, no hospitalizations, and remarks I left blank. Also I didn't change any information like phone number or address.

My original diary was 5 to 7 years and I got the form after 5 1/2 years, this is my first CDR. Also I am 51 years old if that counts any. I am really nervous about this whole process, I have serious problems with anxiety and depression to begin with but this is really pouring it on me. Do you think my form will be kicked out? What are the odds anyone? Thank you in advance, my nerves are shot and any helpful advice would be much appreciated!

When I received my last disability update report (short form - continuing disabilty review) I put the following on the 3 lines for my last 3 "reasons for visit": medication, counseling, prescriptions.

I didn't put my original diagnosis or any diagnosis at all on this particular form.

I had been having psych appointment every month and I did list those dates.

Anyway, about 4 weeks after mailing it in, I received a letter telling me my disability benefits would be continuing.

Anyway, that's my story.

Similar to Walkingtaller I put PSYCH Treatment for my last three appointments... on my short form that I received in February.

Personally in 2010 I got the dreaded long form (MY FIRST CDR) and then was referred to their doctor for consultative Examination. It was scary and frustrating as I had been in monthly treatment for Bipolar Disorder and Anxiety. Needless to say from August 2010 to Nov 2010 following the form and the Con. Examination until I received the letter stating my benefits were continuing was a stressful period, so trust me when I say, we all get it and sympathize.

Just like you I received the short form this time and just like Walkingtaller I put not the diagnosis but "PSYCH TREATMENT".

As of 3/8/14 I haven't' received anything back and I sent it in on 2/16/14.

As long as you are receiving active treatment and your condition hasn't improved and you aren't earning substantial income you should be fine. Even if its pulled for whatever reason the rules are on your side.

If mine gets flagged or randomly pulled I have all my documentation, appointments, and prescription scripts ready and I recommend everyone do the same thing.

With all this being said, chances are you will be fine, but let us know how it works out!

SSDI,
I just sent mine in last month. I put psych, pain management and psych for mine. My original disability is neck and back. SS has got to know that people arent gonna be seeing, like in my case, my ortho doctor all the time. There comes a point when the doctor cant do anything more, then off you more surgery.
Personally, i believe seeing your family doctor on a regular basis, should be just as important, because they really are your lead doctor and will send you to where you need to go.

If SS wants to know just doctors you go to for treatment for your original disability, then they should state that and not hold it against you if your last 3 visits to doctors werent directly related to your original disability.

Thanks for your answer. Apparently what I put down was okay because I just got a letter saying they would not be doing a review at this time. Glad thats over, it was my first CDR, I learned alot and will be better prepared next time, emotionally that is, so stressful I thought I was going to have to go through the whole approval system again! Thanks everyone for your support!

Linda7, often doctors' offices and clinics have a Website with a patient portal. If you've registered there, you can have access to your patient portal and see the dates of any doctor's appointments you've had, plus quite a lot of other information.

If not, you can call the doctors' offices and ask.

ssdirecipient and walking taller, it's been a while since I had to do one of those CDR forms, and I recall that they were always very long but that was years ago. Just speaking off the top of my head here, but I should think that a record showing that you're still under a doctor's care would work to your advantage in establishing your continuing disability. And putting down that you saw the doctor for prescriptions or counseling certainly should indicate that you're still under a doctor's care.

Some people have been diagnosed with a potentially disabling disorder but choose to ignore it and not go to doctors' appointments or take their meds. It is to your advantage to make it known to Social Security that you're not like that.
