Default Form I guess it was the short form ?

Hello everyone please bear with me I am new and new to disability and been getting it for 3 years well its mine time I guess for them to review me and now Im scared. I will loose my benefits and this has totally put me in panic mode and anxiety mode and well major non-sleep mode and major counting day mode and no trust mode and well my head is spinning mode and has just made my mine wander because I have fought so hard for this and then to loose it scares me the reason. I say this is because I do not have the same doctor for the first time when I won my battle I know if I did I would not have to worry for sure but because my spouse is military and we move around its different doctors all the time anyways this doctor that I see yes I see him and have been seeing him for 3 years because as soon as I won my SSD I move 2 months later to a new state and been seen this new doctor at this new state and have build a relationship with this new doctor but its touch and go I see 2 doctors in this office and been going every week I go to counseling once a week and been doing that believe it or not for 10 years and well this doctor here is touch and go I do not trust him he goes against his words sometimes he tells me all the time you do not have to worry but I do not know I feel so sick. I told him I got a paper in the mail he said please stop thinking about it I even called his office in tears crying before my appointment and said I needed a additional appointment not like they would not get paid they said they could not not help me and to stop calling the office anyways my paper I sent went out in the mail on June 13th and Im so scared in the paper it asked me if my condition has gotten better since 2013 or worse this is from memory is that the short form does anyone know???? and then it ask me if me and my doctor talk about going back to work????? And ask me if I have worked????? is that the short form?????? Also after sending the form off which i sent how long does it take to get answer back from them on weather or not my benefits will continue because Im scared and been pacing the house nonstop and so sick to my stomach I want to throw up I would like to know this is my first time with this and any advice would be wonderful as the reason I ask this I have a other military move coming up in January 2016 and want to know if anyone thinks I would hear from them before then of course I can not hold off the move my husband has to move but I has the spouse due to this situation might have to firgue out something Im so sick right now counting down days

yes that is the short form i got a short form also in June about a week before you and have not gotten anything back yet. When i got a short form 3 years ago and sent it back, i got a notice in about a month. I think they are sending more of them out now, and heard from someone that it took two months for them to get a notice concerning their short form after they sent it back.

It has been taking some people as long as 9 months to get a response. It took me 6 months on my last one, which was a year ago.

Your therapist is right, please try to put it out of your head. SS will send you a letter stating benefits will continue or they'll want to do a Long Form CDR. Have you seen a psychiatrist recently? It's a good idea to see one every 6 months and have your meds/dosages evaluated. It seems like you're really struggling and maybe that can be reduced.

It's very uncommon to not have benefits continue, and then it's usually because the beneficiary stopped going to their doctor and/or started working again. And even in those scenarios, they can appeal the decision to have their benefits stopped.

I go to counseling every week I see my doctor who gives me medication every month so I am faith in those appointments I promise you since I know if Im not on medication I am a totally different person and things do not end up well at all and I get my levels check on my medications done on me to with blood work also every 3 months I am faithful on all of this I would be going to group therapy if this military base offer this but it does not so that is why Im on a move to a new a base. Its just when it comes to SSA mail does not get forward and then its hard to get your mail that is why Im asking these questions thank you again I guess Im in the waiting game thank you again.

As soon as you move, notify SS of your new address right away.

Thank you I truly had the best care ever in Washington D.C. area when my spouse work at the Pentagon it was amazing care that. I got then as we have moved it has gone down hill sad and gotten worse not better I do understand cut backs and then when. I the shooting took place at Fort Hood Texas the first one and I was involved it changed my life got exceptional care again and then it went down hill again after we moved again and I stuck it out since they could not relocate us did everything in my power and even try a compassionate reassignment and it was deny every time so now Im on to a new duty station start the begin of next year couple months into the new year and I will say we shall see how it goes. I'm already planning to get a address before I arrive there because they get me house before I get there since I also have more medical problems beside mental health. I have have RSD, and back injury and a prosthetic leg so I try to get housing before I get there due to limited use of stairs thanks again

Thanks for all the help is there any Prosthetic Leg or people who have lost limb groups in this forum at all??? I know about the RSD one already so thank you on that just curious thou I have been dealing with the lost of a limb for only a short time its still adjustment

Sorry, it's a "Neuro" related site, but I'm sure you can find a comparable site for those issues as well.

I hope you're not experiencing spread from the surgical site!

I just got the response to my short form from SSA that they would not be reviewing me at this time. It took 42 days from receiving the short form to receiving the response. Only about 8 days longer than 3 years ago.
