Default 2016 benefitamount form ssa-4926-sm

Hello all im having a hard time to figure out where to post things I have a question I receive SSDI Benefits and my daughter is 17 now and receives benefit under myself now I received a SSA-4926-SM form that we receive every year it tells us about the 2016 cola and how much we will received more and what ur benefits will be for 2016 and the one I got in the mail on 12/10/16 for my daughters benefits it just tell you there will be no COLA this year and your month benefits will stay the same with a break down of monthly benefits and any deductions but I never received my form to keep for my records now 5 days later is their a website that anyone knows of that will tell u information about this form because search SSA/Gov website id said nothing about this SSA-4926-SM(1-2016) anyone with information let me know thank you
