Default Form 455

Good morning all!
New here - first post - that I've started - been on SSDI for almost 10 years. Never been reviewed in any way. Decided to try PT work. I worked for 25 years FT so I received a healthy amount in SSDI benefits.
Now DH is on the liver transplant list and we need to pull in extra money and I found a PT (very PT job) that I can do - 14 hours a week - under the monthly hourly amount - under the monthly dollar amount.
This has a triggered the Form 455-A short form - which has already been filled out WITH remarks and a little confused as to whether I should've used that section.
I have been compliant with SSDI and advised them and supplied them with payroll information - and also have a DR's note that says I cannot work over 4-5 hours a day.
Should I include that Dr's note with my form to SSDI Form 455-A?
It's in the mailbox now - I know that the SSDI is behind in correspondence - I am one of those that is considered MINE (not expected to medically improve) I'm also going to be 60 this year.

Thoughts> Advice?

TIA!

I did a quick search here and found this past thread with quite a few replies on same topic.
SSA-455-OCR-SM Questions

There are more threads, but the system does not allow linking to the full list of results..

I found this board through quick links and the Google machine.
The information is conflicted - so you can see that I'm a little confused.
I *think* I'm on *ok* but ya never know - I brought attention to myself going back to work - its such a push to even work the 4 hours - but it's very light and my employer is very understanding about my condition and my doctor has been very helpful - I have to take extra pain meds to help me through the work day.

There is NO WAY I could work a full-time schedule 5 days a week - I'm unsure what I was actually approved for - my husband is my Representative Payee and even for that I'm unsure why - I had to go in for psych evaluation and also a medical review before being approved on the first pass in 2006. My first check was in 2007.

I tried to figure out the numbers on my approval letter - I had an additional surgery in 2010 - and after the revision surgery in 2010 - I was rear- ended by a semi-truck in a six car pile-up where I was the first car hit by the semi-truck - that caused me to have PTSD and migraines as well as a mild TBI.

Things have changed but I live with it - because what choice do I have???

Default I just read the scan info

I guess I'm low on the totem pole I'm 0097 so I guess that's low in the whole scheme of things.
They just want to check in since I've haven't been reviewed for almost 10 years.
But, working PT might trigger things? It still hasn't been 9 months - so I'll quit before I lost my benefits - there's been a flurry of letters from SSA regarding work and the first letter say I won't lost benefits because of work, then this Disability Update Report after I sent in all the pay-stubs.

I guess I'm just paranoid.

Most likely just a follow up, they just have to verify that all is legit.

How come I can't change the original post to show that I was incorrect in stating MIE - It's MINE - NOT expected to medically improve?
Hmmm - is it because I'm new?
I actually had an account years ago and I have no idea what my user name was nor PW!

New members do have some site limitations for a short time.
