Functional report?

Kinda reminds me of my first time i filled out paperwork....

Found it very strange there looking for people I know that have seen my seizures?....with address and phone numbers?
I dont go out much or due much because of them....
I find the form kinda strange...

Anyone else got one of these latly....was a few years last time i got something to fill out....this one was alot of pages and questions.
