Disability is related more to what functions you can, or cannot do for work, more than the diagnosis name..

Make a list of what affects you and why you cannot work at any job..
to get you started..maybe..
Pain levels - avg & after doing activities
fatigue levels
mental
emotional
can't sit for X amount of time, need to move around , not able to lift x
