Default Short form disability update report

I have been permanently disabled since 2015 from a severe big wave surfing accident... I am here to ask if anybody has any knowledge on filling out the :SHORT FORM disability update report... specifically questions 3 and 5... from what I have read.. I am "CODE" L... "NOT LIKELY TO IMPROVE" which is the case... just want to make sure I don't get flagged and my life line of payments is not interrupted... any help would be appreciated

You have to remember before they would do anything extreme, you will be asked to come in and see one of there drs....I have had 3 of those forms, and seen there Dr. once.( 17 years) I have permanent brain damage, and I do find it a bit insulting that we all know nothing will change. But they still send it!
