Default SS questions

I called SS last week for info and ended up - filing a claim????

Anyway they asked me all sorts of questions and told me to guess when I didnt know somethingand I could change the answers in my phone interview??? Is this true?

What do I claim as a disabilty? I have MS with IBS, Migraines, Vitamin B def. etc. Should I just put all my ailments as symptoms? My biggestes problem is bone shattering pain that leaves me in the fetal position.

Do I just put down my neuro? Rheumatologist?

I originally stopped working when the stress of my job nearly cost me to lose my baby. I was going to stop further along to be a stay at home mom.

I was told that i needed form 13368 filled out. I was told that I could find it on their website?

What should I expect from this interview? Should I postpone till I have all paperwork and answers?

By applying for Social Security Disability Insured Benefits (based on your payment into the insurance program through payroll taxes), or Supplemental Security Income benefits (welfare), you are stating you have medically diagnosed condition that is so severe that it is expected to result in death, or is so severe that it will prevent your performing any type of work (not what you used to do, not what you would like to do, not what would pay a lot of money but any work) that would offer gainful employment.
This is the Social Security Administration definition of disability. It is doubtful that Vitamin B deficiency will meet that standard. Be prepared to list the frequency and severity of migraines, bone shattering pain, and IBS. A calendar listing frequency, what you were doing before the headache started, how long it lasted, and what you did to relieve it will be most helpful. If these conditions have not been severe enough for you to seek treatment don’t bother.
How does your MS affect your ability to perform work related functions? Are you wheel chair bound, do you require crutches & braces, etc.? With physician statements as to the medical need for the assistive devices your case to be adjudicated and allowed overnight.
Don’t be surprised, however, if your application is denied. Your statements strongly suggest you do not have, or even believe you have, a medical condition that meets the SSA disability definition as you indicate you quit work for reasons entirely unrelated to any disabling condition.
If you truly think you have a medical condition that meets the SSA definition of disability you should list every doctor who has treated you for the medically disabling condition(s). You do not need to wait until you have all the paperwork because you will be given plenty of time to modify your application.

For Each Dx that you have, provide doctor's name, address and phone #. Include all diagnoses and match with all doctors treating the condition. Also, for each Dx and corresponding doctor, be ready to name all medications or treatments. Also, have your work history for the past 10 [I think] years. I am treated for depression, and I was lucky that the local Mental Health and Mental Retardation Center which treats me, filled out my computer form, but I came prepared with the above information. Also, where it asks for additional information you wish to provide, briefly say what areas of functioning, such as work, each Dx impairs. There is more, but if you come prepared with the above, you should be able to provide the other info pretty much off the top of your head. I think you also need your SS card and birth certificate, but you might call your local SS office and ask about this, as I can't actually remember. Best of Luck, and Realize that it's not as complicated as it seems, especially if, like you, one has an "interview" which sounds like they are going to help you with the forms. When the second questionnaire came in - this one mainly about work history, I took it to SS office and they helped me fill it out, as it was a paper form, and I have essential tremor and can barely write. I listed all my dx'd conditions, with doctors' addresses and phone #'s, and treatments matched to each. Where there was an opportunity for extra rem arks, I stated how each condition impaired my functioning, e.g. spondalolesthesis impairs my ability to vaccum, rake, sweep, mop and sit for any length of time; depression impairs my ability to initiate and maintain activity of any kind; essential tremors impair ability to write and type; etc. I was approved first time, within 4 months. I take no credit. It was thanks be to God and to the MHMR agency who, due to His influence, helped me fill out the computerized form. You are correct in saying to list ALL conditions. billie
