Help Mri results

I recently had an MRI of my lumbar back secondary to pain. I've been a Cardiac ICU Nurse for 15 years and now work as a forensics/psychiatric nurse in a state hospital.
The results were as follows: diffuse disc desiccation t/o.
disc height loss t/o.
L2-3 end plate sclerosis with disc bulge
L3-4 disc bulge with focal peripheral tear to the posterior margin of the disc bulge without frank herniation. no canal or foraminal narrowing.
L4-5 Advanced disc height loss quite a bit of end plate signal change, also posterior bony ridging with minor disc bulging. Facet joint hypertrophy, more on the left than the rightwhich produces left foraminal narrowing. No convincing mass effect on exiting L5 nerve root but correlation with L 5 radiculopathy is recommended. Right foramen is wide at this level and there is no central canal stenosis.
L5-S1 there is additional very advanced endplate degenerative signal changes with moderate annular disc bulge without frank herniation or canal stenosis. Bilateral foraminal narrowing without obstruction to the exiting nerve roots.
It is suggesting Clinical Correlation. Not only do I have severe back pain, I also have severe Left Hep Pain, walk with a limp, and my left oleg turns out when I walk. My hip/pelvis MRI showed nothing. PLEASE help.
Coulod these findings suggest the reasons for my pain?

How good is your left knee reflex ?

and what does "t/o" represent ?
