Default New conservative approach

INTRODUCTION

Try to fight against the real cause of TOS

Before starting, read this to understand next steps :

Traumatic TOS (rsi ou accident) : explanation based on docter ellis's work :

http://www.doctorellis.com/TOS_neurogenic.html
http://thejns.org/doi/pdf/10.3171/SPI/2008/8/4/347
http://www.doctorellis.com/Lecture.html

So : Trauma -> inflammation spread on the nerve system -> de-differentiation (degeneration so the nerve become immature).

Facts : Regular heparin injection dramatically reduce symptomes but with reccurence

Regular epo injection dramatically reduce symptomes without reccurence

POSSIBLE TREATMENTS

From the most natural drugs to the most "dangerous", but all this stuff is way more safer than surgery

Potential efficacity
: *** : maybe very good potential efficacity, ** : maybe potential good efficacity, * : maybe potential Little efficacity
Danger/side effect : !!! : dangerous !! : risk of medium side effect ! : just be careful

Part I - Anticytokine therapy : less inflammation, less pain and more regeneration

These important pro-inflammatory substance called cytokines (tnfa, nkbba) are at the start of the inflammatory cascade. Pro-inflammatory Cytokine reduce neuronal regeneration and exarcerb symptoms. So we must reduce first pro-inflammatory cytokines for less pain and more regeneration because nerve eletricity (pain) and inflammation reduce this regeneration.

General recommendations : Less stress, more exercice, less sugar and more sleep :-)

hight dose of omega3 is safe and reduce the pain of TOS ***!

(Natural omega3 si safer than synthetic : see the last link)
The inclusion of omega-3 fish oils in the diet has been shown to help with autoimmune and inflammatory diseases (Kelley DS 2001; Simopoulos SP 1999, 2002) by suppressing synthesis of TNF-alpha (Endres S et al 1989). Vitamin E and fish oil work together to decrease pro-inflammatory cytokines, including IL-6 and TNF-alpha, in mice (Venkatraman JT et al 1999a,b). Omega-3 fish oils have been useful in patients with a variety of inflammatory diseases, including rheumatoid arthritis and atherosclerosis (Simopoulos SP 1999). Studies in humans with rheumatoid arthritis suggest that fish oil and vitamin E decrease inflammation in humans (Tidow-Kebritchi S et al 2001). Moreover, fish oil supplementation has shown anti-inflammatory effects, including decreased use of anti-inflammatory drugs, for patients with a variety of other chronic inflammatory diseases (Simopoulos SP 2002).

N-acetylcysteine *!

N-acetylcysteine (NAC) is a well-known antioxidant that also helps regulate production of inflammatory cytokines. In one study, NAC modulated IL-6 production through an NF-kappa B mechanism (Shibanuma M et al 1994). Its is very safe

http://www.ncbi.nlm.nih.gov/pubmed/19555676

Plant extracts **!

A number of plant extracts have also been shown to decrease NF-kappa B, including stinging nettle extract (Riehemann K et al 1999); helenalin from arnica flowers (Lyss G et al 1997); a spiroketal compound found in chamomile and Plagius flosculosus (Calzado MA et al 2005); oleandrin from oleander (Manna SK et al 2000b); resveratrol from grapes and other fruits (Manna SK et al 2000a); 1’-acetoxychavicol acetate (ACA) from Languas galanga (Ichikawa H et al 2005); curcumin (Jobin C et al 1999); ergolide from Inula britannica (Whan HJ et al 2001); rocaglamides extracted from Aglaia (Baumann B et al 2002); and tetrandrine from Han-Fang Chi, a Chinese herb used to treat rheumatic disorders (Ho LJ et al 2004). Try to find some and add it to your diet

Gamma-linolenic acid *!:

In Borage Oil, Black Currant Press, Evening Primrose.
A number of studies have suggested that it reduces inflammation and produces improvement in patients with rheumatoid arthritis and on nerve conduction velocity. This can realy help on the long run

Using decorin, an anti-fibrosis agent, to improve muscle recovery after injury ***(!?)
De-activating TGF-beta (fibrosi)
Fortunately, researchers at the University of Pittsburgh have been working hard to develop ways to control fibrosis. The Pittsburgh scientists realised that fibrosis is stimulated by the overproduction of a chemical called transforming growth factor-beta (TGF-beta). TGF-beta is produced by tissues in response to injury and disease and is the major cause of fibrosis in humans and other animals.
The Pittsburgh researchers also knew that decorin - a proteoglycan (a protein with sugar attached) naturally found in the human body - has the ability to de-activate TGF-beta. Decorin has been shown to inhibit fibrosis in the kidney, liver, and lung, so why - the Pennsylvania scientists reasoned - would it not also stop fibres from taking over muscle tissue?
In recent research, the Pittsburgh medical team has in fact been able to show that decorin is a powerful anti-fibrotic agent in muscle tissue - and thus that decorin administration has the potential to be an outstandingly effective treatment for muscle injury (Kazumasa, F. et al., 'The Use of An Antifibrosis Agent to Improve Muscle Recovery after Laceration,' The American Journal of Sports Medicine, Vol. 29 (4), pp. 394-402, 2001).
These results suggest that decorin not only prevents fibrosis but also improves muscle regeneration and repair.

angiotensin II ***!

By modulating the response to local and systemic angiotensin II, angiotensin receptor blocker therapy significantly reduced fibrosis and led to an increase in the number of regenerating myofibers in acutely injured skeletal muscle
Angiotensin receptor blockers may provide a safe, clinically available treatment for improving healing after skeletal muscle injury.

EXTRA

Serrapeptase (***?)!

Serrapeptase is a safe homornal drug and is usefull for carpal tunnel syndrome, so maybe for tos too !!

Serrapeptase clears out the inflammation and dead/scar tissue. By clearing this problem tissue, it enables the nerve signal to work unimpeded and possibly the body’s own healing system to replace it with healthy tissue.

Here a detail of the CTS study :

Here a study for CTS with Serrapeptase (same problem as TOS but only on wrist)

This study was planned to assess the response of Serrapeptase in
patients with carpal tunnel syndrome (CTS) ====> Same mechanism as TOS !!

Twenty patients with CTS were evaluated clinically. After baseline
electrophysiological studies, these patients were given serrapetase10
mg twice daily with initial short course of nimesulide. Clinical and
electrophysiological reassessment was done after 6 weeks.

Mean age was 43.9 years with male to female ratio of 1:2.33. Sixty
five percent cases showed significant clinical (13/20)
improvement which
was supported by significant improvement in electrophysiological
parameters. Recurrence was reported in only four cases. No significant
side effect was observed.
===== > 9/20 were saved with no reccurence and the 4 case can try again + very rare side effect not like surgery !!

CONCLUSIONS: Serrapeptase™ therapy may proved to be a
useful alternative mode of conservative treatment. Larger study
may be further helpful to establish the role of Serrapeptas in CTS. Don't know how but it work !
