If there's no message on the phone at the office , call the hospital he was associated with, maybe they have that info??

Usually a letter is sent out when, or before a Dr retires or moves, telling you who will have the files.

I was told by emily that they mailed them to me. I never received them when I called back for them right before the office closed they wanted 50.00 which I did not have.
Anyone that did not request records I was told they were shredded. I fyou have any luck let me know, PLease

As far as I know if you have not kept from court cases, 0r had letters sent to other providers then there is little chance they still exsist. Some of the files were extensive, I know my dd]s would take a huge box if I had needed to pick them up,. We do not need any. My last appointment in April the previous file was not located.
So if you need something for disability, or w/c see if your other treating doctors, or attorney may have some in their storage that was sent to them by Dr. T

if anyone still has Dr. T's email could you PM me. I talked to him about doing video's but I got wrapped up with work claims and time slipped by..

When he retired from active practice years ago the Med surg sent letters. But then he started counsulting practice, really pt, pt the last several years.
Are you sure that he did not send letters to your primary care doc of his findings that are in you PCP file?

What about letters to W/C or attorney's? I do not even know how to reach Emily now. Like I found out, when I went in this year, my file from 2001 could not be found. It had been put in storage at some move. He had been in Kingston, then at Mercy Health care offices.

Did you try Dr. Nakkachi's secretary to see if Emily is reachable. Maybe she can do a search for you.
