I've had the Botox shots. During my 1st round with TOS on my right side, I opted for surgery after the 1st injection stopped working at 1 month after the shot. I am still symptom free on that side but it is my non-dominant side. I developed TOS on the other side from a shoulder injury and have had 3 or 4 shots into that side. The doctor warned me that Botox is less effective as you get more shots. I think he mentioned something like 4 to 6 shots. I'm still opting for conservative treatment as my symptoms are, unlike the 1st time, somewhat manageable at this point.
