Yes, often after surgery on the more symptomatic side, the other side will start to show more sx.

I have bilateral arterial & neurogenic thoracic outlet syndrome. Yes, it's possible. My symptoms manifested approximately 4 months apart from one side to the other.

I have bi-lateral TOS. The right cervical rib was larger that the left, so there are/were more symptoms on the right side.

I started out with bilateral, that is what confused doctors the most because it was on both sides and they were clueless. It can spread as well, because it started going into my legs and feet once I got off my medications.

My surgeon did my procedures 4 months apart. Felt 'right' to me.

I imagine it could be likewise with the legs (upper body always hurts - the brain causes the legs issues?), but if I were you, I'd check out the possibility of arterial blockage in the legs, low spine problems, or some other problem. Many people with TOS have additional dx.
