Default Daughter with TOS - surgery decision soon

My 20 year old daughter was diagnosed with TOS in Fall '13. Had an acute episode where hand and forearm turned cold, blue, and severe pain. Spent 4 days in hospital. Pain management, MRIs, vascular studies, doppler, EMG, ENG, -Raynaud's tests. Has been doing PT and extremity exercises and stretching. Shoulder and arm still pretty uncomfortable at times. Hand still has some color changes and turns cool, but transient. Has regained more strength in hand/arm. She'll meet with him next month regarding decision on surgery. Her vascular surgeon does not do the surgery but refers out to larger centers.

Looking for recommendations for TOS surgeons in our region: MD, OH, DE, PA, VA, NC. Willing to travel to get her the right surgeon who has lots of experience doing this surgery.

Suggestions for surgeons in states mentioned (OR those to avoid).

Have the drs mentioned which kind of TOS she has?
NTOS -nerve
VTOS - venous
ATOS - arterial
Combo of the above
disputed - misc / chronic repetitive

I hope the PT/physio is very skilled with advanced training, even better a TOS specialist, and not just doing the standard protocol for generic injuries.. weights & over stretching, pushing thru pain does not work for us..

Delayed pain and trigger points are big obstacles that need to be factored in.

How did she acquire TOS? sports, musician, accident etc.
