Default Very best doctors for TOS?

I know all the doctors are posted up above, but if you guys could pick the top 5 doctors for TOS (diagnosing), who would they be?

Diagnosing only, or treating & surgery also?
Location not an issue?

The surgeons that do TOS and related surgeries for the top pro ball players must be quite good, a lot of team money riding on a successful outcome..
They must be good at dxing also..

Default im starting from scratch and willing to go anywhere.

I need proper DX as well as treatment, and I'm willing to go anywhere. Just need some names to start making calls and setting up a game plan.

I would highly recommend Dr. Robert Thompson at Washington University/Barnes Jewish Hospital in St. Louis. I found his diagnostic process to be far superior to anything I found in my local area, and I felt very confident having him perform the surgery I had in 2012 for treatment of my vascular (arterial) TOS. My surgery was successful and I am very pleased that I decided to go there.
