Make her take you to the doctor!!!
Tam, you know how many of us say that taking hubby to the docs is a good idea because hearing it directly can get the message accross so much better...could you use this technique with your mom? would she be willing to take you to the doc appts(even grudgingly)?
Also, maybe you could set up a standing list and have vons or albertsons deliver your groceries on a schedule every week- just some basics that you know you will need...? Include a rotisserie chicken or some other pre-cooked items and you'd have some easy food. You can set lists like that up online,m and not have to re-enter them each week. I am sure you know this, but you have been so distracted maybe you have forgotten...
personal relationships are SO hard...i am often up and down with my own mom and daughter. Can you get just a claening person to do laundry and clean for you while you look for, or even while you have, in home help? They are, i think, easier to come by than good in-home help, and might offer just a little consistency, and take some pressure off of df.
