Default TOS Surgery and therapy info

We used to have a info thread on surgery- so I thought I'd try to do a poll on it.
There is room for many more options {up to 100} here,
multiple choice click on any that apply.


I meant to have a No surgery option- but I can't add it in now.

But if you don't select any of the surgery choices then we will assume you had no surgery and are using other methods.
