Default migraine journal\documentation

hi everyone,

I need to find out from anyone who keeps a journal of sorts for their medical docs and more important for their disability carrier to show the history of what you experience. pain, headaches, cognitive lapses etc...

I have kept calendars, on line text docs but they are hard to keep up with especially when you are experiencing the headaches is the worst time to try to document the event and then how much information is crucial vs too little. I was advised to start sending a monthly report to LTD but not sure what to include and to avoid the trappings of tmi. I cant afford to purchase any fancy software but would like to know from someone who has a method and works for them in this situation

Multiple injuries - here for support of mtbi, chronic headache and cognitive deficits.

Hi jinga,

I've never done it for myself although I probably should have after my first head injury.

I figured there would be some ready made journals online so went looking.

There is this one

This one is a "Wellness Chart"

They may be too detailed though.
Trying to find one specific to migraine.
Many of the headache/migraine only ones are very complicated.

This one is a monthly migraine journal though that doesn't look too convoluted.

There are probably more on TBI/PCS sites. If I find any more I'll post them for you here.

If you have a smartphone, look into the iHeadache app. It is free and very user-friendly. I used it and found it very helpful. http://www.iheadache.com/

mTBI and PCS after sledding accident 1-17-2011

Was experiencing:
Persistent headaches, fatigue, slowed cognitive functions, depression
Symptoms exacerbated by being in a crowd, watching TV, driving, other miscellaneous stress & sensory overload
Sciatica/piriformis syndrome with numbness & loss of reflex
