Hello I have crohn's, and had surgery to remove my TI many years ago. My body does not seem to absorb B12 at all from oral supplements. I still take a B12 complex daily, and I seem to do ok with absorbing other B vitamins just not B12.

I am having problems with B12. At the very least I have to take an Injection once a week or I have severe issues. Lately I am noticing that this is not enough. I do exercise, lift weights, and I am very active.

I am currently injecting 1000mcg aka 1ml daily for the past 3 days. My symptoms are improving dramatically, rapidly, and basically I feel so much better. It is rather unbelievable, and I don't know how to really explain it.

After reading through some threads here I started taking my potassium supplement again. Potassium Chloride 20meq once daily.

I have been using cyanocobalamin, and my only concern is that I am taking too much.

My g.i. doctor has been no help, but has no problem prescribing me however much I want. I have seen other doctors about this, but have found nobody to be knowledgeable on the subject. My last test showed my level to be around 2000. My Hemoglobin is 168 g/L. My hematocrit is .52 L. My creatinine is 132.6 umol/L.

Before I started doing daily injections my problems were a red beefy tongue. I have a trail on the side that looks like an ulcer sort of, and sometimes it hurts sometimes it is ok. I also have a white coating on my tongue that is not yeast. Anxiety, depression, and mild psychosis check. After doing the injections my problems completely disappear, except for the tongue which I suspect will take much longer, or could be related to another vitamin deficiency such as Vitamin A, and the white coating on my tongue which I have been told is geographic tongue. I have also started taking Beta Carotene.

After reading more information I am seeing that it says on the little pamphlet inside the box of 25 single 1ml vials that I can do injections of 100mcg daily instead of the full 1000mcg. I am wondering if this would provide the same benefits, and if anyone else has tried doing this?

My last B12 labs came up 2000 and I take methylcobalmin. I passed on all labwork with this year's annual check in as I'm working on issues that I needed to and I'm doing better.

I take many supps for the past 25 yrs and my favorite supp is grape seed extract. I've never done research on grape seed and crohns.

B12 injected remains in the serum for about 72 hours. So injecting every day may not be necessary. A study showed about 150mcg absorbed from 1000mcg injection. So using 100mcg may not work well.

I would get the DNA testing for MTHFR mutations. If you have this problem, the cyano form would not be a good choice for you.
Methylcobalamin or hydroxocobalamin may be needed instead.
10-30% of people have this mutation and cannot activate cyano to its active forms.

If all of your terminal ileum is gone, you will have to use injections from now on.

Thanks for the replies. Yes my T.I. was completely removed along with some small bowel, and colon. The surgery was done long ago. I have tried the sublingual B12 as well, and it does nothing for me.

I also take Humira, and believe that it interferes with B12, Iron, and Magnesium. I have to supplement with iron bisglycinate daily and magnesium chloride several times a day or else I don’t feel right.

I have some questions about the other forms of B12.

I found a few places on the internet for methylcobalamin injections. Any leads as far as where to purchase?

What is an equivalent dose compared to cyanocobalamin or conversion.
Does 1ml of methyl = 1ml cyan?

The methyl form is less stable than cyano when it is removed from the multidose vial. It has to be protected from light, and heat once the vial is punctured.

This is one way to protect methylcobalamin injection sources:

If you do decide to inject methylcobalamin, you'd need this type of storage for it. Perhaps a local compounding pharmacy would sell you some (an affordable small number) for your use.

You do have to consider WHO you purchase from:

Hydroxocobalamin which may work for you, is available on RX and a pharmacy can order it for you.

So a doctor's prescription for this form can be ordered by any pharmacy for you.

Getting the DNA testing, will reveal if you can use the cyano, however, and save $$ in the long run and convenience. If you are not heterozygous, or homozygous for the MTHFR problem.

B12 is not really absorbed sublingually... as you discovered. But historically it has always been available and so the companies still make it. The B12 dissolves in the saliva and is swallowed to be then absorbed in the terminal ileum.

The doses are the same for all 3 types of injections.

So just be careful from whom you purchase your methylcobalamin injectable from.

It safe taking 1000mcg of b-12 per day even when your b-12 levels are normal?

Yes, it is. Only 13micrograms or so are really absorbed into the body from that dose orally. The rest is eliminated in the stool.

There is no upper limit of toxicity for B12 yet.

I am in Asia and found a product called Tribimin which is injectable.

It is B12(cyan) + B1 + B6. I have been taking this instead of the B12. It is helping me more than the B12 alone was. I have since moved to .5ml injected daily. I am going to continue doing this for a while as it is helping me.

I also take a B vitamin by Aleve, and my multi has B vitamins. Plus my Magnesium supplement has B6.

Is it possible to also not absorb B6 or B1 very well orally?
Are there health concerns with daily B6, and B1 injections?

You'd have to research the terminal ileum for information about
what is absorbed there. All nutrients have different situations involving absorption and where in the GI tract this happens.

B1 seems okay so far in high dose injections. This is actually becoming a treatment for Parkinson's. Our PD forum has threads about it.

The B6 however, should be dosed carefully. Some people react to high dose B6 with nerve symptoms. Some recommendations are to use only a maximum of 100mg a day orally. Some people get symptoms as seen on this board, at 50mg though. Not all of the oral dose is absorbed however, like most things in the GI tract... so I don't really know the full potential.
This question has never come up before regarding injectable B6.

Thank you again for your reply. I will be more careful with the oral B6 since two of my magnesium tablets alone have 50mg B6.

More information reading from the glass ampule.

Each 3ml ampule contains

B1 – 100mg Thiamine Hydrochloride
B6 – 50mg pyridoxine hydrochloride
B12 – 1mg Cyanocobalamin

Directions for severe cases – 1 ampule should be given daily. For after treatment 2-3 times weekly.
