The Function Report is VERY important.

Consider what you're able to do, not able to do. What you've been required to adapt, etc. It will be compared to medical documentation and needs to be consistent with report of functional limitations. So, don't exaggerate, but don't under report either.

The Disability Report is obviously very important as well. List any medical conditions that prevent you from working. So, it's better to not list conditions like adult onset diabetes that is controlled with diet and/or meds, for example. List them from most severe to least severe. It's important that you're receiving ongoing medical treatment for these conditions and are compliant with your treatment plan and taking prescribed meds.

Your Alleged Onset Date must be after you stopped working at the SGA level ($1090 per month in 2015). If you have any complicated history tring to choose that date, ask questions.

Plan on filling out a copy first to practice on and then when perfected, transfer answers to originals sent by SS.
