List any medical conditions that prevent you from working. So, it's better to not list conditions like adult onset diabetes that is controlled with diet and/or meds, for example. List them from most severe to least severe. It's important that you're receiving ongoing medical treatment for these conditions and are compliant with your treatment plan and taking prescribed meds. Your Alleged Onset Date must be after you stopped working at the SGA level ($1090 per month in 2015).
