Unfortunately (or fortunately, depending on how you look at it), there's no easy way to make something you do on your computer available on the web. Access is not a database supported by any web host. You might consider, instead, using something like Google Apps, as you can put forms tied into their spreadsheet in order to gather data.
The real question, though, is what's the purpose of gathering this information? We don't allow any kind of research on NT. Best, DocJohn
