Either a noticeable worsening of present symptoms or new ones forming. Though even that can be a little deceitful, because the symptoms might not be noticed until it’s heading back into dormancy.

I think an MRI (with contrast) might be the only way to tell especially if symptoms are too light to be noticeable.
