Confused How far does reality slip

okay so i will make this fast so it does not run on to much, any further info please contact me. At 14 as a healthy normal male i was in a car wreck where i was out for several weeks had severe head injuries, broken jaw in 3 places, spleen ruptured etc.... I had no quality kare after the hospital release. i went home and remained without to much follow up at all. I complained of issues but was dismissed. i can "see" differently after that day which is hard to explain. but overtime and looking back things that have been obvious had i had any help or knowledge are now costing my life a great deal and my reality seems to be slipping away. some of them are a ringing that has sense the last five years grown to a continued tone, head aches come and go, i get dizzy when i close my eyes, have for a long time. but what i see that others dont is the inner mental functions. I forget phone numbers and passwords, have violent episodes brought on by fear and confussion, cant be around public places, write and say words with misplaced letters, and way to much to write here. The point is this, how can i or can i at all be looked at by a doctor. does anyone maybe have advice on things or just anything that can help me. For many years i have noticed things growing worse, even my phyical body has done unexpected things (i had an inflamed pancreas at 24 years old?)
