My heart goes out to you, MH, I sense that you have spent a large part of your life helping others, and now you don't know how to get the help you need, and maybe don't even know what help you do need. I hope you will try every other physical therapy before choosing surgeries, I have not had one myself, but have known others, and so many false promises are made....
When i had the whiplash, i received Acupuncture for it, and that was so much better for me than pain meds, which had terrible side effects,,, on my "constitution" and my memory. Now I am facing a degenerative spinal disease and the future is going to demand that i learn pain management techniques and how to force myself to remain active, which it sounds like you have done. Well Done! Congratulations for all the work you have put in to finding ways to retain your quality of life! Hope to hear more of your ongoing progress. Best Wishes Gus
