We have to develop better imaging techniques so that we can get a better feel of what is going on in mysterious structures in real time. I guess, like in Alzheimer's, PD can begin by destruction of unique areas of the brain and expand outwards as progression occurs. Also, this may show the spread to autonomic nervous system control, lower in the brainstem. Eventually, loss of specific neurons which control breathing takes a lot of us down.
