I know the class of abx which Levaquin and Cipro are in, have done a lot of joint/muscle damage to folks I've heard from over the years. You might look at that.

I have a friend in a class action suit of Lipitor (statin) which she took for many years and has a lot of nerve damage primarily in feet.

I have a couple toes on my right foot (right side of my body is from hip replacement mess) that start to pain and burn now most nights. I can only think it's all related to the surgery I went thru in 2010. When this comes on I get up and rub my feet and add a topical solution I use, I use many and finding tea tree oil helps too. I usually get back to sleep but if I think it might take some time I take an Inositol. Oh, I was going to do some research on Inositol and neuropathy. Most days NOW I take 750 mg Inositol part of this is with my sleep combo. C
