Most of the PNers here keep their B12 levels at 1000.
I would start with 5mg methylcobalamin on and EMPTY stomach and see if you experience improvements after 3 months. I have more information on a stickie on the PN forum. It is full of links and good information. If you do choose to try methylB12 stop it for several days before and further testing. Otherwise testing will be falsely elevated.
