Default RSD guidlines

I just watched a disturbing program on PBS called "Under our skin-A healthcare nightmare." It's about lymes disease and the hurdles many victims go through with doctors who have to follow guidelines issued by insurance companies. Many of these advanced cases are treated like many victims with RSD---Poorly. Many doctors who have been successful treating Lymes have been sued by the insurance companies because they do not follow their guidelines. Some have lost their practices. I advice everyone to try to see this program.

It seems to me the most successful American doctors who treat RSD do not except insurance. My question is: is there a set of guidelines pertaining to RSD used by insurance companies and if so does anyone know what it states?
