DEAR FRIEND,

This survey will be of BENEFIT TO YOU and others suffering with CRPS/RSD. We are interested in what YOU think.
Be candid and opinionated if you wish. All surveys will be read.

Please complete this survey if:

you have been diagnosed with CRPS/RSD
you suspect you may have CRPS/RSD
if you have been diagnosed with fibromyalgia in the upper extremities
if you have symptoms similar to what is described below and no diagnosis.

I went to the trouble of filling this out and the validation code was messed up and blew it away.

Default Hi Sandel and Imahotep,
I had to fill it out twice and mess with the validation code 4 times before I finally got it to take it. I typed in exactly what I saw in the code box and it still took 4 times.

It is a good questionaire to fill out. I hope everyone will do it.

i filled it all out and the validation code was screwy twice! so it erased all my answers. i will try again later.

Thanks! I filled it out, and had no problem with the validation code. TIP: You have to fill in the validation code and submit it BEFORE you begin filling out the survey.
I filled it out even though I don't meet the IASP criteria for RSD! I guess I fall into the mysterious "other/ fibro in upper body" categories. I wonder why they are including oddbals like me?
