There are lymph nodes that go up the neck from the nape, on both sides. They can become infected/swollen if something is going on in the scalp. I have seen them enlarge when kids have head lice. Ringworm may also do it. Or a staph infection in a hair follicle. Sometimes some drugs, or illnesses like Cushing's syndrome, can cause a fat pad back there.
