Default breakthrough help

Hey there. It's Coleen in AZ. Been quite a while since I've posted. Good to see some old names that are familiar. I've had diluadid in my pump since 2005. Works well but not so well lately. In fact that's why I'm writing. For the last few weeks I've been in the 9s - you know what I mean. Mostly bed-ridden can't do much. For breakthru I've been taking oral dilaudid and neurontin but I've been having to double dose the dilaudid, to minor effect. OK so my question: Is there anything out there that would amplify my dilaudid? I was thinking dextromethorphan but I wanted to ask you guys first. Hoping you guys have some ideas for a revised pain cocktail- my worst is the afternoon.
