I have been receivng benefits for three years and now it's time for Social Security to review my status and determine if I still qualify for benefits. I completed the first set of paperwork they sent me but now they want me and my wife to fill out function reports-one for me and one for her to fill out. (Third Party) Do these tips apply to my case as well? Or do you have any special tips for me? Thanks!

In your case, you are undergoing a Long form CDR and were also sent the Function Report and your spouse sent the 3rd party Function Report. The above advice should be fine, but the main issues are going to be has your function improved, stayed the same, or decreased since you were originally approved? Have you attended school or training or worked since being approved? Have you continued seeing your doc/s regularly and been compliant with prescribed meds and treatment? Were you sent the short form CDR first??? Here are a few links about CDRs: http://nymakesworkpay.org/docs/Guide_CDR.pdf http://ssdfacts.com/forum/index.php?board=6.0 |
