Default A personal journal for continuing reviews?Do you?

Hi everybody! My husband suffers from seizures, depression, anxiety and has a arthritis condition.His reviews are every 3 yrs.

My question here is- Do you or should you keep a personal journal at home of your condition and the good days & bad? Is it required at review time? And does it help/hold up at review time? Or does SSA just listen to what ones doctor says?

And if you do keep a journal is your daily, weekly, year by year? How should it be done?
