Yes it was..I just thought they Wanted all Drs.so Vists to chiropractor took up alot of my space that should have went to my disability.I go to my Neurologist 4-5 Times a year so i wondered about attaching a sheet of paper.But from what ive read thats not good.but the form just gets me through Jan 2011 I'm suppose to go to Oct 2009..
