I think the same thing is going to happen to me. I believe it is a formal review. I think it happens to most everyone about 6 0r 7 years in. Get all your records together, and contact the person that helped you to get your dissability. I had a lawyer assistant, whos only job it is to help people with their dissiablity. This is what I am planning to do. I stopped taking an Opiate, and went down to a much lower strength medication. I worry they will consider that getting better. I also have other issues as you do. I have spinal fusions too, with plates C3-7. I do have a letter from my doctor stating I am not better, but that this is a permanent condition. I am not sure exactly how they do formal reviews. I bet someone will come along and say what happens next. ginnie

if you get the short form, only check the answers for the questions asked. dont write in the remarks section at the end.
when i did mine, the quesiton for the last three doctors visits, i put the visits that had to do with the illnesses i receive ssdi for. so i just wrote the date and what i went there for. for example 12/14/14 Peripheral Neuropathy
that s it. dont write anything extra, dont attach anything. dont have doctors send anything in.
the long form is where you must go into greater detail. writing anything extra on the short form will lead to it getting kicked out by the computer and a possible long form review.

Most people get the short form CDR. If you fill it out correctly, regularly see a doctor, your health remains the same, you aren't working or going to school, you shouldn't be required to fill out the dreaded long form...
