Default Hi New here, question on CDR, nervous

Hi All,

I am new here, but not new to forums. I have a question, okay maybe a few, LOL, and I'm having anxiety! I did try searching your wonderful posts and can't quite find the answers.

Today, I received my first CDR in the mail! As if things weren't tough enough already and I feel the same anxiety I felt while applying. I got approved in 2010 and got retro pay back to 2007. I received the short form and I know it's read by a computer after I fill it out. Also, I've read to not put any new info on it, like my last surgery a yr ago, unrelated to my disabilities, as it will throw up a red flag.

So, they are asking for my doctor appts, what it was for and even if for prescriptions since February 2011, starting with recent first. They give you 3 lines for 3 appts. Any more, you can add it under the remarks section, 5 lines.

Now in one year, I've had many appts and, every single month, I have to pick up my scripts from the doctor. How do I do this, I just saw my primary today for a physical, Thurs called my pain doc for my scripts, a few months prior saw pain doc for my every 3 month "check my pain levels and to see if I'm worsening". I have severe DDD of the cervical spine, 4 spine surgeries, Depression, Asthma and Post Concussion Syndrome, and anxiety.

How do I write every month of the past yr and a half, picking up my scripts? How do I fit in all my different appts? So sorry, please bear with me, It will take up the lines in the remarks section! Not to mention, the computer will more than likely kick it out to a human, then it opens up getting a long form!!

I want them to know I take my meds religiously since day 1 and I do go to my follow ups, but in excessively writing that all in the remarks section, I know the computer will kick it out! Only because everything else is to be in caps and in those blocks for the computer to read!

Please help Me, after the battle to get approved, I need to do this right.

Thank You so much if you can help me! I'm really terrified of messing this up!

just put down the last three appointments that pertain to the medical condition or conditions for which disability was awarded preferrably specialists but gp if necessary.
dont write in the remarks section!

THANK YOU! Quick question? Tomorrow I see my DO for physical and we go over my anxiety and depression issues, which I'll put. I had my pain doc appt in November just his every 3 month check up on my severe DDD(SHOULD I spell it out or put DDD?), which I will put, but what about my monthly prescriptions? In between the two, should I write one visit as "monthly prescriptions"? So they know I am taking my monthly painmeds? Again, THANK YOU FOR YOUR HELP! My anxiety is now full blown mode and I keep checking on here! God Bless, Angel

Me again, LOL, my mind is in overdrive right now! My follow up with my Pain doc/Physiatrist every 3 months, includes Neuro checks for any worsening symptoms. I think I'm just going to put "cervical spine disease" instead of DDD. The form gives examples, like, bad back! My approval, one part says, Severe Disc Degenerative Disease including Cervical spine, and that's what I see him for every 3-6 months. How do shorten it to fit in 15 or less boxes and get the point across? Does that make sense?

Anyway, THANK YOU again, and I'm sorry for being a pain. It took me so long to fight to get approved and it almost did me in, this too....

just put the date and under reason for visit what condition or disease that you were approved on as the reason and only as much of it that fits in the boxes. when you run out of boxes stop writing even if the word is cut off. dont change anything, list the conditon as they list it

for example for me it would be

09/22/2012 peripheral neuropathy
11/17/2012 asthma
1/20/2013 peripheral neuropathy

thats it. no need to list for medication or follow up or anything else just list the condition

if you have 1 condition that you were approved on you canlist last three visits in time period, if two, then last three pertaining to two and if three conditions, one visit for each condition.

Thank You from the bottom of my heart! The main thing I was approved for was, "Severe Disc Degenerative Disease & of the Cervical Spine", I've been trying to figure out how to write that in those few boxes. LOL! That's what's stressing me out tbh.

I guess I just start to write "disc degenerative disease". Obviously it's severe, so I'll leave that word out, but dealing with computers, you never know.

Again, Thank You for helping me. I will leave out the monthly prescriptions and I am not going to write in the remarks section for sure. Had you not helped Me, I would have!

Wow, just looked at my approval again, and this is how bad my anxiety is, it's actually,"Degenerative JOINT disease involving the Cervical spine"! Geez, this is why I stress myself out! I do have DDD, but I didn't get approved for that, it was o f the joints & cervical spine! Also, asthma, depression and post concussion syndrome! The anxiety didn't matter.

Yes, I am a mess!

you have company, if i was a horse they would shoot me.

so to recap since you were approved because of multiple illnesses or conditions put the last appointment pertaining to three of them. just dont go back so far as to go past the time period they are asking about.

also dont worry about the question as to wether you and your doctor discussed returning to work. if you havent discussed it just put that down and dont sweat it.
