yes, but I let her finish filling it out for me because I was freaking out and she didn't stay inside the boxes, and didn't put what I saw dr's for she listed the doctors and wrote outside of those boxes, the rest were yes and no boxes and now I will probably get the long form in the mail......I was approved after being denied the first time and I got lawyers and then I was approved and I didn't have to see a judge because he wrote that he reviewed and at my age and the lack of jobs, there would be no way I could get a job at my level , but I was listed as could possible get better, then I got the short form and I didn't know what that meant...I thought that was the start of the whole mess all over again........is there anybody that messed up on the short form and they called u for information or sent u another short form, I mean part of my diagnosis was a cognitive disorder from an aneurysm I had coiled and things just don't make sense to ma a lot of the time

Have you mailed it yet? If not, than I would suggest you call your local SS office and ask for a replacement. I believe they send out another copy if you fail to respond as well.

Getting the long form isn't ideal, but if you are still receiving medical care and your condition hasn't improved, you will likely not have a problem, but it is more work.

You saw this link? http://nymakesworkpay.org/docs/Guide_CDR.pdf

yes but I saw it too late, I panicked when I got the form and called my nuerologist but he ws not in so I call my psych and was in such a major paic she said she would help me out and she mailed it this past month, there is no way to get it back is there ? I want to cry and I can't sleep and I see an ENT in the morning for a terrible fall I took, haven't been using my walker......I did send an email to the disability lawyers I used last time to get ssdi in the first place, I was supposed to see a judge, but he wrote our a letter as to why I should have full approval, will that come in handy to use? I didn't have to see him
I am sorry if I don't make sense
