Default Disabled since 2006 and Update report ssa-445-ocr-sm

Hello
Little about me. I got approved in 2005/2006 for social security disability for my profound deafness and speech recognition below 40% without any hearing or denied claims.I got approved right away submitting proof and going to the SSD doctor for hearing exams and my problems were caused by bacterial meningitis.

I got my first Disability update form ssa-445-ocr-sm this month and my question is about the 3 most recent doctor visits. since feb 2011. Do I put down that I saw the doctor for headaches or heartburn issues? I made a visit with the ENT for my ear pain and pressure caused by the nerves.

My deafness is never gonna get better and I'm sure it will get worse sooner or later, Do I need to put that in the form or SSD already knows that my problem will never get better.
