Okay Echoes, i pulled out my reviews from 2007 and 2010.

For the visits to my psychologist i wrote down PSYCHOTHERAPHY as reason for visit

For visits to my psychiatrist i wrote down PAIN MGMT PSYCH

Those were on last 2 cdr's i filled out.

now my family doctor was the middle visit.

I cant decide between writing down MEDICINE ADJUST OR REGULAR CHECKUP

Which one sounds better to you? Am probably driving you crazy now,lolll.

I just want this to be over so i can get back to trying to get my heart and diabetes problems helped. I wore the holter monitor last week and my heart stopped 77 times during the night, from 2-6 seconds each time. So this paperwork sure isnt helping.

if it were me i would put down the last 3 visits then with the psychiatrist and gp, because they are the ones directly treating the condition you are receiving ssdi for, and the diagnosis. if you want you could put pain mangagement and checkup if it makes you feel better. im only going by what new york state is telling people here to do. are you discogenic, degenerative or both?

Bobby maybe you could call them and just ask a general question about how to fill it out? They must get asked this question a thousand times a day, there are 50 million people on SS and I'm sure they get questions all the time. I don't think it would flag your account even if they did ask for your name and ss #, all you are doing is asking a simple question about how to correctly fill out the form. Just a thought, good luck.

Hi ECHOES.
I have both. Discogenic and degenerative.

I have read that ny state guide like 10 times now and am still confused. Doesnt SS count going to your primary doctor as part of your overall treatment of any disability, considering he treats your overall care and sends you to doctors you need to see. You would think SS would want to see you going to your primary doctor every 3-6 months as part of keeping up with your total health.
Since the form isnt asking for doctors names or what they do, if i use my primary doctor has my middle visit, what would be the best wording to use for reason for that visit? checkup, 3 month checkup, regular checkup or any other ideas?


I see my psychiatrist on friday, i could then use the order of visits like this

PSYCHOTHERAPHY feb 2014

PAIN MGMT PSYCH october 2014


Thus throwing out the visit to my primary doctor.


I guess the biggest reason am confused and having a hard time with this, is because what kind of treatment SS wants to see for the condition i was approved for? I go for pain management and get medications, i get counseling for the problems the pain is causing. I go to my primary doctor and cardiologist all the time to maintain my overall health. Even if i went to my ortho doctor, hes just gonna wanna know if i want more surgeries to fix my other discs. other then that, hes gonna push me off to my primary doctor or pain management to treat my pain.
Theres really nothing more i can do with my neck and back, is live with what i have or if it gets to bad, more surgeries which nobody wants.

I swear if this paperwork doesnt give me a stroke or put me in the metal ward, nothing will,loll.


Echoes, thanks again for the help. Its great to have somebody in the same boat to ask questions to.

SS,
I actually thought about doing that, but then i thought, sure they will a little not in my file, telling the computer to spit my cdr, so that they can check my doctor visits.

Heres the thing, am not lying about anything. I have got so many different added conditions now, they should really be paying me double,lolll. I just dont want the hassle of having to deal with this, as i already spend a ton of time in doctors offices. I just want this form to get thru with no problems, like am sure all of us on SSDI want to happen.

i think that you are over thinking it. just put your last 3 visits dates with psychiatrist then if the gp isnt handling back/neck and reason for visit what you are getting ssdi for. you arent getting ssdi for a psychological reason so i dont see how psychotherapy is a direct treatment for neck back issues. it might help you to deal with it but it isnt a direct treatment for it. it might result in your short form being kicked.

Thanks Echoes.

I see the psychologist for counseling of the problems my pain has caused me from my injury.
He treats my PTSD, depression, anxiety, panic disorder all effects caused by my injury. Sure hes not treating my spine, but he is treating what the injury to my spine has caused. I dont get hurt, i dont have those problems. They have gotten worse as the years go by. The PTSD still gives me nightmares of the day i got hurt.


Iam so confused as to what to do, when this should be so simple. I wish that i was back at work fighting fires, instead of disabled doing this paperwork crap.
One of my last doctor visits was to cardiologist for stress echo test, but i dont dare put that down because its a different problem. SS knows my neck and back problems arent suddenly gonna heal themselves after 13 years. You would think putting down other doctor visits like seeing your primary doctor on a regular basis for checkups, would show you are trying to take care of your health. I guess am looking at the form how i would review it if i was SS. Like with your condition, i wouldnt penalize you if on one of your last doctor visits, it wasnt for your approved condition.
They should put down in the instructions, that they want a persons last 3 visits only to doctors that treat you for your approved condition.

Okay Echoes, my final question before i send the form in tomorrow. I have studied it way to long now. When i started going over this form last week, i was 51, iam now 67 years old,lolll.

The question how is your health now.

better - same - worse

Truthfully my health is worse right now and i wouldnt be lying if i checked that box, but i would be willing to just check the same box, if it put me at a lower risk of computer spitting it out.
am sure checking better would spit it out, do you think worse will do the same thing?

Thanks, am gonna put worse also, because iam. Will just have to cross my fingers and hope it goes thru without problems. Then i wont have to worry about the next review for 7 years and at that point it should be a breeze.
