Help Help with CDR

Boy as easy as the short form CDR really is, i still never know how to fill it out.
I have been on ssdi since 2001 and have had 3 reviews already, so i should be an expert, but am not.

My disability is from my neck and back

my last doctor visit are

last visit was to my psychologist for counseling not part of original application, but still is connected to my injury

second to last visit was to my primary doctor for checkup. should i write checkup or psychical checkup

third visit was to my psychiatrist who i go to for all my mental problems associated with my injury. He also manages my pain and gives me pain meds.

fourth visit was to my cardiologist for a stress echo test {have major heart problems, but not part of my application}


My question basically is, my last visits to doctors werent to my orthopedic surgeon but they were all to doctors i go to on a regular monthly basis, who help me with the problems associated with my injury, but werent a part of my original application.
Its not like i can plan ahead perfectly for a trip to the ortho, so that visit would fit in as one of my last 3 visits to a doctor.

My wife says am just way over thinking this and making it harder on myself then it really is. Shes probably right, but still cant help not stressing over this and not wanting to make a mistake and have it get spit out of the computer.

Any help with this, would be of great help to me.

also to add to my last post, which of these would be better wording for the computer

1. neck - back medicines
2.pain management
3 pain medicines

My lean is to number 1 because it refers directly to the body parts my case is about.

am driving myself crazy with this stuff and have only gotten a couple of hours sleep each night for the last 5 days because of this form.

anybody who has an idea which wording to use, your input will be so greatful to me and my wife {driving her crazy,lolll}

i would put for wording the name of the condition or conditions and the last 3 visits that have to do with those condition or conditions even if you had other doctors visits for other reasons after them. in other words put last 3 visits that are relevant for reason you are on ssdi.

for me it would be as follows

Dr. so and so 2/25/14 peripheral neuropathy
Dr. so and so 1/5/14 Asthma
Dr. so and so 10/26/13 peripheral neuropathy

thats it.

Default Check out my thread with a similar question

When I received my last disability update report (short form - continuing disabilty review) I put the following on the 3 lines for my last 3 "reasons for visit": medication, counseling, prescriptions.

I didn't put my original diagnosis or any diagnosis at all on this particular form.

I had been having psych appointment every month and I did list those dates.

Anyway, about 4 weeks after mailing it in, I received a letter telling me my disability benefits would be continuing.

Anyway, that's my story.

Echoes thanks for the help.
Am kind of screwed then, because i havent seen my ortho surgeon in a few years has he has said there is no reason to see him anymore, unless i need more surgery. All the other doctor visits are basically to help with my neck/back injury, but they arent the doctors that did my surgery.

Am wonder if i should just keep the last 3 visits like this

1.counseling
2.medical checkup or physical checkup {which sounds better}
3.pain medications or pain management


am ortho doctor has already gone on record that iam permanently disabled and i have tons of medical records to backup all my health problems which currently are neck/back, afib, sleep apnea, high blood pressure, diabetes and mental problems. I just dont want to have to deal with not getting this thru the computer, as i already spend enough time in doctors office,lolll.

Also Echoes where are you writing down the doctors names on the form. My form only asks for reason for visit.


Iam probably worrying more then i should be as my profile type is L and my profile score is 0039.

Thanks for the help and please let me know, if you think what i wrote up above would be good enough

Thanks SS for the answers. I kind of like how you did that. You kept it short and sweet when filling out your paperwork

i dont have the short form in front of me so i wasnt sure if they asked the doctors name. if it were me i would just put the last 3 visits of the doctors that are currently helping you with your neck/back injury and the diagnosis that you are receiving ssdi for. if you have more than one condition which social security listed as the reason you were approved then make sure to list a visit for that condition also. it doesnt have to be the surgeon who did the operation it could be the doctor that is currently seeing you for you listed condition. you do have pretty low numbers so i wouldnt sweat it too much. if you click that link you will see that the state of new york advises to only write you condition for reason for the visit.

Echoes, this is where am getting confused. My last 3 doctors appts were to

1.psychologist, who helps me with coping with my injury and what is has done to my life

2.psychiatrist, who also does the same, but also provides me with all the medications that help with my disability, including pain pills, muscle relaxers, pain patches, sleeping pills


3. my primary doctor, which i see every 3 months. since i no longer see my surgeon, isnt he really considered my main doctor for all my health, including my injury?


all 3 actually help with the symptoms of my disability and have been providing the things i need.

If these doctors arent good enough, then am basically screwed because the last time i saw my surgeon was a few years ago. What we talked about is if i wanted to do anything with the bad discs i still have. He told me, that if i couldnt take the pain anymore, to come back and he would do more surgery.
Well i dont want another surgery until i cant take it anymore.

Man this 1 question has got me so stressed out, my head feels like its gonna burst.

In fact on the form, they give examples of what could be written down.
heres their examples

HAVE YOU GONE TO A DOCTOR OR CLINIC FOR TREATMENT {INCLUDING EVAULATIONS, CHECKUPS, COUNSELING, PRESCRIPTIONS, MEDICINES}


This is why i thought for the psychologist i put down COUNSELING

for the psychiatrist i put down PAIN MEDS SPINE

and for primary doctor i put down CHECKUP.


Since they are only gonna see counseling, pain meds and checkup with no doctors names, they would have to look at those 3 visits as having to do with my case.

Or do i just write down

neck/back
neck/back
neck/back

for all 3 visits?

Maybe i can win the mega millions tonight and then call them up and tell them to keep their money.

Echoes if you have any more thoughts, am all ears, as i know you have been doing this a long time and your info is always good

how is your condition worded by social security? in other words on your award letter what did they say they were awarding you for exactly? if you look on your short form you can see what all the codes stand for.
