i would put for wording the name of the condition or conditions and the last 3 visits that have to do with those condition or conditions even if you had other doctors visits for other reasons after them. in other words put last 3 visits that are relevant for reason you are on ssdi.
for me it would be as follows Dr. so and so 2/25/14 peripheral neuropathy Dr. so and so 1/5/14 Asthma Dr. so and so 10/26/13 peripheral neuropathy thats it. http://nymakesworkpay.org/docs/Guide_CDR.pdf |
